Phillip L. Forni, DPM, graduated from The Scholl College of Podiatric Medicine at Rosalind Franklin University in North Chicago. After completing his foot & ankle surgical residency at Northlake Hospital, Dr. Forni started his career in Oaklawn, Illinois, and eventually with AFP Podiatry LTD., in Harwood Heights, Il. 60706. In 1994, the Roselle office location opened, creating a second office site for AFP Podiatry LTD. Dr. Forni has hospital affiliations with Alexian Brothers Medical Center in Elk Grove Village and Community First Medical Center in Chicago (northwest side). Dr. Forni is a double board-certified physician and holds multiple professional memberships.
